<p>Perbedaan sebenarnya pada perputaran reproduksi laki-laki dan perempuan adalah bahwa laki-laki menghasilkan sperma baru setiap harinya sementara perempuan terlahir dengan jumlah telur yang terbatas dan tidak terbarukan dalam ovarium mereka. Karena kualitas dari telur terus berkurang sejalan dengan bertambahnya usia, maka wanita yang sudah berusia diatas 35 tahun lebih sulit untuk hamil. Disisi lain, laki-laki menghadapi masalah yang berbeda. Tingkat kesuburan mereka amat dipengaruhi oleh kualitas, kuantitas dan pergerakan spontan sperma mereka. Kegunaan organ-organ reproduksi mereka, seperti testis, skotrum dan epididimis (terletak dalam pembuluh), juga memiliki pengaruh.</p>
<p>Kekurangan makanan dapat merusak system reproduksi. Zat seng, asam folik dan kalsium, yang merupakan mineral yang dapat ditemukan dalam makanan, harus selalu menjadi bagian dari diet sehari-hari untuk mempertahankan fertilitas atau tingkat kesuburan. Konsumsi juga A, C, E dan selenium, yang dalam jumlah cukup berfungsi sebagai anti oksidan, untuk melawan radikal bebas dari rokok, alkohol, obat-obatan dan lain lain.</p>

Sewaktu berat badan bertambah, maka  bertambah pula tekanan pada sendi lutut. Bahkan aktivitas biasa seperti  berjalan atau naik turun tangga bisa menyebabkan banyak beban pada  sendi. Hal ini juga meningkatkan risiko osteoarthritis dengan  mempercepat rusaknya tulang rawan. Cidera pada lutut juga bisa terjadi  karena digunakan terlalu sering, olahraga atau aktivitas fisik, tidak  pemanasan dan perenggangan tubuh sebelum olahraga, masalah dengan posisi  dan trauma (karena kecelakaan, jatuh atau pukulan langsung sampai  lutut).</span></p>

Cidera ligamen awalnya bisa diobati dengan sekantung es,  penghentian pergerakan, dengan cara beristirahat dan peninggian. Jika  kondisi tidak bertambah baik dalam tiga sampai tujuh hari, sebaiknya  kunjungi dokter atau spesialis di ilmu kedokteran olahraga atau  orthopedi. Selain memberikan terapi fisio dan memberikan obat yang tidak  mengandung steroid dan tidak menyebabkan peradangan, pasien kadang  diberikan penjepit atau papan penyangga untuk melumpuhkan sendi untuk  mengurangi rasa sakit dan mempercepat kesembuhan. Tetapi, secara  tradisional, fisioterapi dan pharmacologic terapi kerap tidak efektif  atau meninggalkan sisa-sisa gejala. Karenanya, penyuntikan suplemen pada  lutut (</span><em><span style="font-size: x-small;">Viscosupplementation</span></em><span style="font-size: x-small;">), pembedahan Arthroskopi (</span><em><span style="font-size: x-small;">Arthroscopic surgery)</span></em><span style="font-size: x-small;"> atau pencangkokan tulang  rawan sebagai jaringan penghubung pada lutut (</span><em><span style="font-size: x-small;">Articular Cartilage  Transplant</span></em><span style="font-size: x-small;">) dapat dilakukan untuk memperbaiki cidera parah.</span></p>
<p><strong><span style="font-size: x-small;">Viscosupplementasi</span></strong></p>
<p><span style="font-size: x-small;">Viscosupplementasi adalah  pilihan baru yang ada bagi pasien dengan gejala lutut osteoarthritis,  yang melibatkan rentetan injeksi intra-artionlar asam hyaluronic.  Sementara, pasien yang tidak menyukai pengobatan tradisional sebaiknya  mencoba perawatan ini. Suplemen Hyalgan disuntikkan secara langsung ke  dalam sendi lutut untuk memperbaiki gizi dan pelumasan. Pada kebanyakan  kasus, pasien menemukan kenyamanan dalam berjalan setelah injeksi.  Tetapi, penting untuk memperhatikan bahwa Viscosupplementasi ini  biasanya dilakukan jika semua jenis pengobatan lain telah dilakukan  namun gagal untuk mengurangi rasa sakit.</span></p>
<p><strong><span style="font-size: x-small;">Pembedahan Arthroskopi</span></strong></p>
<p><span style="font-size: x-small;">Pembedahan Arthroskopi  merupakan prosedur orthopedik yang praktis dan cukup dikenal untuk  mendiagnosa dan mengobati masalah sendi lutut. Pembedahan Arthroskopi  dilakukan dengan memasukkan kamera ke sumber cahaya serat optik ke dalam  sendi lewat torehan kecil. Ahli bedah memanfaatkan aliran air  bertekanan rendah untuk membersihkan lutut dan menyingkirkan bahan yang  mengandung racun, melonggarkan badan atau memperbaiki jaringan atau  membuang jaringan yang tumbuh secara berlebihan menggunakan laser.  Pasien hanya diharuskan tinggal semalam setelah pembedahan dan sudah  bisa kembali ke rumah tanpa bantuan.</span></p>
<p><strong><span style="font-size: x-small;">Pencangkokan Tulang Rawan</span></strong></p>
<p><span style="font-size: x-small;">Tulang rawan adalah bahan  yang paling mengagumkan, kuat, elastis dan awet yang melindungi ujung  tulang di sendi dari tekanan. Kondisi idealnya, bisa bertahan seumur  hidup. Tetapi, jika tulang rawan menjadi rusak, malah aktivitas yang  paling biasa bisa menyebabkan erosi tulang rawan dari permukaan sendi  dan jika tak diobati, kemungkinan akan sangat menyakitkan, kehilangan  fungsi, lumpuh dan dapat mengarah ke osteoarthritis lutut.</span></p>
<p><span style="font-size: x-small;">Selama beberapa tahun  terakhir, pencangkokan tulang rawan lutut sudah menjadi metode yang  berhasil karena dapat memperbaiki sendi lutut yang cedera terus-menerus  akibat olahraga atau kecelakaan. Prosedur pencangkokan ini di rancang  untuk menumbuhkan kembali tulang rawan pada sendi lutut yang rusak  akibat trauma atau radang sendi. Tekniknya melibatkan memanam tulang  rawan si pasien dan memindahkan jaringan tersebut ke area yang rusak,  melalui satu prosedur operasi.</span></p>

Biasanya, struktur  kompleks sendi lutut ini bekerja bersamaan secara halus untuk memberikan  keluwesan dan dukungan pada tubuh, serta pergerakan yang lebih luas.  Akan tetapi cidera, usia lanjut dan penyakit bisa mengganggu  keseimbangan ini – mengakibatkan rasa sakit, kelemahan pada otot dan  berkurang fungsinya.</span></p>
<p><span style="font-size: x-medium;">Olahraga  berat dan aktivitas berulang seperti naik sepeda beberapa kilometer,  joging tiap hari, tertekuk dan terpelintir tajam dari berolahraga  berisiko tinggi mengakibatkan jaringan otot penghubung aus dan  menyebabkan tekanan ekstrim. Jika tubuh tak dapat pulih dari ketegangan,  hasilnya akan menyebabkan peradangan, sehingga ligamen rusak. Menurut  para spesialis, “hanya sedikit orang yang tahu bahwa melakukan pemanasan  sebelum dan sesudah latihan bisa menambahkan kekuatan untuk tubuh,  membantu pencegahan cidera dan mempercepat waktu kesembuhan”. Karena itu  penting bagi kita untuk disiplin mengikuti garis pedoman pelatihan dan  menghindari menggunakan kelompok otot yang sama secara berturut-turut.  Otot yang tegang atau lemah mengurangi dukungan terhadap lutut karena  lutut tidak dapat menyerap dengan cukup tekanan pada sendi lutut. </span></p>
<p><span style="font-size: x-medium;">Orang dengan kelainan  struktural tertentu misalnya satu kaki lebih pendek dari yang lain,  lutut tidak sejajar dan kaki datar, cenderung lebih banyak terkena  masalah lutut. Trauma yang disebabkan oleh kecelakaan, jatuh atau  pukulan langsung pada lutut bisa menyebabkan cidera pada ligamen di  berbagai lokasi pada bagian atas lutut, bagian luar lutut atau di dalam  lutut itu sendiri. Luka pada bagian tersebut kebanyakan terjadi karena  rasa sakit yang berlebih, biasanya nyeri pada saat tidur, bengkak, rasa  seperti terbakar dan biasanya diperburuk dengan menekuk lutut dan  berjalan. Dan sewaktu kita bertambah tua, kondisi lutut seperti  osteoarthritis, encok dan pseudogout (penyakit pengendapan kristal  kalsium pirofosfat dihidrat) menjadi hal yang biasa.</span></p>
<p><span style="font-size: x-medium;">Cidera ligamen awalnya bisa  diobati dengan sekantung es, penghentian pergerakan, dengan cara  beristirahat dan peninggian. ?	Rektum (saluran akhir usus besar) tidak terhubung dengan lubang anus.<br />

<p>Faktor penyebab terjadinya atresia ani belum diketahui secara pasti:<br />

?	Merupakan (kegagalan perkembangan) anomaly gastrointestinal (sistem pencernaan) dan genitourinary (sistem perkemihan)<br />
?	Kelainan kloaka pada saat embrionik<br />

<p>Kelainan bentuk anus akan menyebabkan gangguan buang air besar.<br />
-   Ketika lubang anus sempit, bayi kesulitan BAB menyebabkan konstipasi dan ketidaknyamanan.<br />
-   Jika terdapat selaput pada akhiran jalan keluar anus, bayi tidak bisa BAB.<br />
- Ketika rektum tidak berhubungan dengan anus tetapi terdapat fistula, feses akan keluar melalui fistula tersebut sebagai pengganti anus. Hal ini dapat menyebabkan infeksi.<br />
- Jika rektum tidak berhubungan dengan anus dan tidak terdapat fistula sehingga feses tidak dapat dikeluarkan dari tubuh dan bayi tidak dapat BAB.</p>

Dia tidak tahu akan akibat dari penggunaan obat-obatan tersebut di masa yang akan datang. Hanya kenikmatan dan euforia yang dirasakan Mr.X saat menggunakan obat-obatan tersebut.</p>
<p>Kini Mr. X (28 tahun) harus terbaring lemah di sebuah RS besar di Jakarta karena penyakit AIDS yang sudah dideritanya selama 5 tahun. Lamanya penyakit tersebut menyebabkan imunitasnya menurun drastis sehingga kuman mudah masuk ke dalam tubuhnya. Kuman tersebut (Streptococcus) sampai ke otak dan merusak saraf-saraf disekitarnya menyebabkan Mr.X sering kejang, hilang ingatan bahkan keluarga pun tidak diingatnya sampai dia juga lupa akan kata-kata.</p>
<p>Setiap hari Mr.X diajarkan 2-5 kata oleh Dokter dan wajib mengingat setiap kata yang sudah diajarkan kepadanya. Mr. X dengan semangat pantang menyerah selalu mau belajar dan mematuhi semua pengobatan yang harus dijalaninya karena dia bertekad harus sembuh dan bisa kembali ke rumah bersama keluarga. Semangat yang dimiliki Mr.X tidak lepas dari dukungan keluarga yang selalu mendampinginya selama dia dirawat di RS.</p>
<p>Perawat juga memiliki peran yang sangat besar bagi Mr.X. Selain memberikannya obat secara teratur, mereka juga memberikan dukungan moral dan spiritual kepada Mr.X agar selalu bersemangat dalam menjalani hidup dan terus berdoa untuk kesembuhannya karena harapan itu selalu ada.</p>
<p>Untuk mendapatkan hasil yang maksimal diperlukan usaha yang maksimal juga. Perjuangan dan semangat Mr.X untuk sembuh dan bertahan hidup tidak sia-sia, Mr.X berhasil kembali ke rumah karena kondisinya yang mulai membaik.</p>
<p>&#8220;Selama masih bisa bernafas teruslah berharap dan jangan pernah putus asa karena harapan itu akan selalu ada&#8221;</p>
